TouchPoint Health has launched a new and improved tool that optimizes care coordination for patients across the continuum of care. The Care Navigator™ Mobile App provides a means of accessing and updating information in the Care Navigator Hub for “on-the-go” care team members and patients. Care Providers are able to engage more closely with their patients and given the capability to view patient information, care plan summaries, and care team members at the touch of their fingertips.
Future updates will allow patients to have more control of their day to day health and wellness and enable their care team to securely communicate while delivering support. This comprehensive tool is on the fore-front of changing the way patients and their care team coordinate and communicate to provide the best care possible.
